Their greatest mistake was to launch the operation in the first place rather than use these forces to hold the front line, or in counter attacks along various sectors when needed. The Ukrainians should be building huge defensive fortifications all along the front line but aren't. Their standard mode of operation seems to be getting themselves into pockets, holding out under fire, taking huge numbers of casualties, and repeating it.
